This two-page staff evaluation form is a quick and efficient tool for assessing paid and volunteer staff performance in a play-centered early learning setting. I developed this form with help from Child Care Bar And Grill co-host Rebecca. This staff evaluation form can be used in a number of ways. For example:

  • Evaluation by a supervisor
  • Evaluation by a peer
  • Self-Evaluation

For a multifaceted evaluation, consider a combination of the above. You’ll get a clearer picture of a staff member’s strengths and weaknesses if they both self-evaluate and have evaluations completed by a supervisor and a couple of peers.

Completing The Staff Evaluation Form

Here’s a rundown on how to complete the form:

Top Matter

Simply enter the name of the person being evaluated, the evaluator’s name, and the evaluation date.

Scoring

Score each item in the three categories with a number score between 1 (does not meet expectations) and 7 (exceeds expectations). Each item also has space for a short note.

The Sections

The staff evaluation form has 3 sections: Professionalism, Relationships And Interactions, and Environments And Programming. Three blank lines have been left in each section, so you can add custom items.

Additional Notes

The form also has a large space for additional notes so evaluators can clarify their scores and provide additional feedback.

Wrap Up

What you do with the results is up to you, but it’s probably best to have a plan before you start completing evaluations. For example, maybe scores of 1 and 2 require some sort of remediation, maybe the development of a plan to improve in those areas–Or maybe staff members scoring 7s in 25% or more of the listed items receive a bonus.
You should also decide how frequently your program will complete staff evaluations–annually, biannually, quarterly?

Whatever you plan, make sure staff know about it before evaluations begin.
