Overview
Use the Minor Injury Notification form to inform parents about minor bumps, bruises, scratches, scraps, and cuts that occur as part of developmentally appropriate active play.
Here at Playvolution HQ, we believe occasional bumps, bruises, scratches, scraps, and cuts are part of childhood. These things occur as children test their abilities and playfully work to strengthen and learn to control their bodies. We also think it is useful to separate these minor, common, and expected injuries from more series injuries. This can be done by clarifying the difference in program policies and procedures and reporting injuries to parents with different forms. This separation makes supporting the active, physical play children need easier because it helps normalize the minor injuries as an expected part of childhood.
This form is smaller and less detailed than a full Injury Report. This helps signal that what happened is worth documenting, but not a serious incident.

Using The Minor Injury Notification Form
Start by printing copies of the PDF and slicing them in half.
When a minor injury happens, grab a blank copy and enter the child’s name and the date at the top. Then describe what happened in the large box. “Mortimer was playing Super Ninjas with Smadar when he fell and scrapped his knee on the asphalt tricycle path. He hopped up and kept playing. When we went back inside, we cleaned the scrape.” Then sign it and give it to the child’s parent at the end of the day.
